This booklet provides a list of topics to be considered and tasks to be completed by a person facing the end of their life. It also provides a space to record all of the information that loved ones and executors may need to provide the end of life care, funeral and estate management desired by the person passing.
When completed, it will contain significant confidential personal information, including passwords, that could be used by another person to access accounts and information. The booklet should be completed personally and without the assistance of third parties, and be securely stored together with a Will until needed.
